Physical Examinations
The Loyola College Athletics Department requires all intercollegiate student-athletes to complete an Intercollegiate Athletics Health Information Booklet and pre-participation physical exam (by a qualified physician) prior to competing in any practices, games, strength and conditioning sessions, and/or any intercollegiate athletic activities. All intercollegiate student-athletes must complete a pre-participation physical and Intercollegiate Athletics Health Information Booklet every year of participation in a varsity sport.
The student-athlete is responsible for obtaining his/her own pre-participation physical exam before arriving at Loyola. The Loyola College Sports Medicine Department will not arrange pre-participation physical exams.
Intercollegiate Athletics Health Information Booklets must be completed and returned by August 1 to the Athletic Training Room before the student-athlete is permitted to participate in intercollegiate athletics at Loyola College in Maryland.
Please refer to the Intercollegiate Athletics Health Information Booklet for a detailed list of medical forms that must be completed.
Any questions regarding physicals and/or Health Booklets should be directed to the Sports Medicine Department staff.
Health Insurance Information
The Loyola College Athletics Department requires all intercollegiate student-athletes to be covered by health insurance before participating in any practice, game, strength and conditioning session, and/or any intercollegiate athletic activities.
All student-athletes must submit a photocopy (front and back) of their health insurance card and prescription benefits card.
Any changes to health insurance information should be reported to the Sports Medicine Department staff as soon as possible.
